Enrollment Process
Enrollment: Please see the Parent Handbook for details about our Enrollment Procedures.
The following forms are required for enrollment. Please click below to print out each form:
1) WCM Enrollment Form
2) WCM Registration Form
3) Proof of Identity (copy of birth certificate, passport, adoption papers)
4) Commonwealth of Virginia School Entrance Health Form (including Immunization record)
5) Co-op Team Affiliation Form
6) Discipline Policy Form
Completed forms need to be mailed or delivered to the WCM office in hard copy. Each child’s file must contain this full set of completed forms.
